Outpatient registration

You may pre-register for your outpatient visit 30 days in advance of your appointment date.  To pre-register, call our central scheduling department at 388-96777 or 1-800-348-9677.  We will need to know the date and time of your appointment and your physician’s name.  Please have your insurance card and doctor’s order in hand when you call. You will also need to bring your insurance card(s) and doctor's order with you.

Once you are pre-registered, you will be given a confirmation number and, in most cases, directions to the department where your procedure will be performed.
