If You Have Symptoms of Heart Failure

Heart failure means that your heart isn’t working as efficiently as it should. It can be caused by many conditions that damage the heart. 

Symptoms of heart failure include:

  • Dizziness, fatigue and weakness  
  • Irregular or rapid heartbeat

If you are Diagnosed with Heart Failure

The Congestive Heart Failure Clinic was designed to provide specialized care for patients with CHF. Our multidisciplinary staff includes physicians, nurse practitioners, clinical nurses, nurse navigators and pharmacists who work closely with you to determine a personalized plan of care to manage your condition. 

In the clinic, you will meet one-on-one with medical providers who are specially-trained in congestive heart failure management. We help you manage your CHF through:
