The release of information office is open Monday – Friday from 8 a.m. to 4:30 p.m. and can be reached by calling (304) 388-1308. Proof of identity is required when you pick medical records up in person (driver's license or some type of government issued photo ID).
You can now access many documents and results related to your care online with PatientLink, CAMC's free and secure patient portal. Visit our PatientLink site to learn more or sign up for access.
If you need information from your medical record sent to another provider, please call (304) 388-1308. There is no charge for this service. (A provider is someone who is providing professional medical care for you. An example would be a specialist to whom you have been referred.) Please provide the name, address, phone and fax number of the physician/provider to whom you want the information sent.
If you need information from your medical record for your own use, please be advised there is a copy charge of 36 cents per page. Please complete the Authorization of Use and Disclosure and mail or bring it in person to the address provided on the form. If you would like records sent to you via email, you will also need to complete the Request for Alternative Communication of Protected Health Information via Email
The patient or the patient's legal representative must sign the attached Authorization of Use and Disclosure. If you are the patient's legal representative, we require proof that you may sign on behalf of the patient (example: Copy of the Medical Power of Attorney papers).
