This provides you with another convenient way to manage your health information from your mobile device. You must have a current CAMC PatientLink account to connect. To request a connection with a mobile app, complete the form below. The requested app will need to be assessed to see if it meets technical requirements. You will receive a follow up within 2-4 weeks.
