What is street medicine?
Street medicine brings medical care directly to individuals experiencing homelessness or housing instability – meeting patients where they are, including streets, encampments, shelters and other community locations.
Community Connect Outreach reduces barriers to care by delivering compassionate, patient-centered medical services outside of traditional clinic settings. Our goal is to improve access, build trust, and connect patients to ongoing primary and specialty care within Vandalia Health and through community partners.

• Connecting patients to primary care, specialty services and social support resources
Who we serve
The Community Connect Outreach team serves individuals experiencing homelessness, housing instability, and other barriers to accessing traditional healthcare services.
Additional services offered
• Evaluation and cleaning of acute and chronic wounds
• Treatment of wound infections
• Dressing changes and provision of supplies
• Monitoring and follow-up
• Referral for advanced wound management when indicated
• Assessment for substance use disorders
• Initiation and continuation of medication-assisted treatment when appropriate
• Bridge prescriptions for medications to support continuity of care
• Overdose prevention education and naloxone distribution when available
• Care coordination and referral to local opioid treatment programs for ongoing comprehensive treatment services
• Collaboration with behavioral health and recovery support resources
• Evaluation of minor illnesses and injuries
• Vital signs assessment and basic health screenings
• Medication refills and management when appropriate
• Referral to emergency services when required
• Coordination with primary care, behavioral health and specialty care

HIV care can be very complex. We have assembled an experienced “care team” of professionals to help with the health-related, social, psychological and economic concerns of our clients.
What Sets Us Apart
The first Ryan White Part C grant recipient
Charleston Area Medical Center has been providing compassionate, quality care to individuals infected with HIV/AIDS since the late 1980s. In 2001, the CAMC Institute received the state’s first Ryan White Part C grant from the Health Resources & Services Administration (HRSA) of the U.S. Department of Health and Human Services.
CAMC’s grant is being used to implement a comprehensive system of outpatient care for individuals in 19 southern West Virginia counties, many of whom are uninsured or underinsured.
